EMPOWERED SUBSCRIPTION ORDER FORM Thank you for your interest in the quarterly publication Empowered! Every issue of Empowered provides information on developmental disabilities. The cost of your subscription goes toward the non-profit organization, TEAM DANIEL FOUNDATION. The purpose of this foundation is to provide resources and supports to individuals with developmental disabilities and their families so they are able to access the services they need. Please complete the below. The cost of the annual subscription is $22.00*. Payment instructions will be given after you've entered your information.
